Rough idle or hesitation when you accelerate
An engine that shakes at a stop light or stumbles when you press the gas is short on one of three things: clean air, clean fuel delivery or a good spark. A fresh engine air filter and a fuel system cleaning are the two we handle when you pull in, and they clear a surprising share of these; plugs, coils and sensors go to a repair shop.

This week
General guidance for this symptom. A warning light that is flashing or an overheating engine: stop driving and call.
Likely causes
- A dirty engine air filter
The high desert is dust, and a filter that has gone 20,000 miles past the Car Care Council's inspect-at-every-oil-change advice chokes the engine at idle first. We pull it, hold it to the light with you watching, and replace it if daylight does not come through.
Engine air filter - Carbon and deposits in the fuel system
Injectors that do not spray cleanly and a throttle body coated in carbon give an uneven mixture, which is a rough idle and a stumble off the line. A fuel system cleaning runs a concentrated cleaner through the injectors and intake and is the most common thing we do for this complaint.
Fuel system cleaning - Worn spark plugs or a weak coil
A plug past its interval, or a coil breaking down under load, misses under acceleration. If the check engine light flashes, stop driving. Plugs and coils are a repair shop's work; we can rule out the air and fuel causes first so the service center starts with a narrower list.
Multi-point maintenance check - A vacuum leak or a dirty sensor
A cracked hose or a dirty mass airflow sensor lets unmetered air in and the computer cannot keep up at idle. These are diagnostic items for a repair shop. We note any obviously cracked hose we see on the bay check.
Multi-point maintenance check - Old fuel or the wrong octane
A truck that sat through a winter in the Ranchos with half a tank of gas starts rough, and a turbocharged engine on 87 when the manual says 91 hesitates on the climb up Kingsbury Grade. Fresh fuel at the grade the manual lists is the fix, and it costs nothing to try first.
Fuel system cleaning
A rough idle is the engine telling you a cylinder is not pulling its weight. Hesitation is the same story under load: you ask for power and there is a half-second hole before it comes. On a four-cylinder at Reno's altitude the hole is easier to feel than on a V8, and it is why a Civic that ran smooth at sea level can feel lumpy at the light on Plumb Lane after a few dusty years here. Start with the cheap, likely causes, which happen to be the ones a quick lube does well.

Cold or warm, idle or load
| When it happens | Points at | First step |
|---|---|---|
| Rough only when cold, smooth warm | Deposits, old plugs | Fuel system cleaning |
| Rough all the time at idle | Vacuum leak, air filter, plug | Air filter, then a repair shop |
| Stumble off the line, fine once rolling | Dirty throttle body or injectors | Fuel system cleaning |
| Miss under hard acceleration or uphill | Coil or plug breaking down | Repair shop, stop if the light flashes |
| Started right after a fill-up | Bad fuel | Fresh tank from a busy station |
What the visit looks like
We start under the hood with the engine running and listen for a hiss that means a vacuum leak, then shut it down and pull the air filter. A filter packed with Washoe Valley dust is replaced on the spot. We check the oil level and condition, because a badly overfilled or very old oil can make an idle rough on some engines, and we look at the fuel cap and the visible intake hoses. If the air side is clean, a fuel system cleaning is the next sensible step and it is done the same visit at any of the four service centers.
If the idle is still rough after those two, the cause is spark, a sensor or a leak we cannot see, and we say so. The invoice lists what we replaced and what we ruled out, which is exactly what a repair shop wants to know before it starts billing time.
Why the dust matters
The Reno airport gets about 7 inches of precipitation a year and Carson City and Minden about 9 (NOAA 1991-2020 normals). That leaves most of the valley floor dry, and every gravel road in Spanish Springs, Stagecoach or the Gardnerville Ranchos puts fine silt in the intake. The engine air filter is the only thing between that silt and the cylinders. Our dusty roads guide explains how often to look, and the answer is more often than the manual printed for a wetter state.
Idle and the smog check
For Reno and Sparks drivers there is a second reason to sort a rough idle: a misfire sets a code, a code turns the check engine light on, and a lit light fails the Washoe smog check before the inspector reads anything else (Nevada DMV). Cleaning up the air and fuel side before the test is cheaper than failing it.

Questions people ask
Still have a question? Call your service center or drive in; there is no appointment to make.
Will a fuel system cleaning fix a rough idle?
Often, when deposits are the cause, which they are on a lot of cars past 60,000 miles that have never had one. It will not fix a bad coil or a vacuum leak, and we will tell you that before you pay.
Can you replace spark plugs?
No, plugs and coils are a repair shop's work. We handle the filter and fuel system side and rule those out.
The check engine light is flashing. Should I drive in?
No. A flashing light is an active misfire that can ruin the catalytic converter in a few miles. Pull over, shut it off and arrange a tow to a repair shop.
Does altitude make a rough idle worse?
Yes. Thin air at 4,500 feet means less oxygen per intake stroke, and an engine with a marginal mixture or a weak spark shows it at idle sooner than it would at sea level.
